This is a good summary.  Basically what you are in the mood for-- in order of effort required.  

D1/D2, or Hou Va type places, minimal effort,  Get in, select, relax, get out.  Lower end of price range.  

Sauna, go to relax for many hours, soak, eat, chill with friends, hang out with girls.  Medium price range (but can go high if you select model class or european/japanese/korean)

KTV (haven't gone in Macau, but have gone in Mainland), very active, higher energy, some game helpful to get the girl in the mood for later.  Much more expensive due to extras being paid for (food, alcohol, time).  Have to negotiate/arrange for OTC session with girl.

Nightclub, I haven't done this in Macau either, but I assume it's like it is all over SE Asia and HK, somewhere between a sure thing and regular approaching in a bar, have to negotiate everything and it really takes a certain mood and certain effort for me.  But, this can often result in a good OTC arrangement, and can feel more like an accomplishment if you find a good one at a good rate.  

Regarding Saunas, my go-tos are Familia Nobre, East Castle, and Rio.  Rio will be most expensive of those.  Golden Dragon was pretty good when I went in the past.  I think it can be more about the time of day you go vs. the place for a good selection.  One other thing I enjoy doing at the saunas is trying to flirt with the massage girls and wait staff and get them OTC, have been successful before but you need to be discreet.
