I usually text something like this:

"Hi, [name of provider].
I'm a new customer. Will you have time at [time] on [date]?
Thank you!
[My name]

If it's an organization, I'd say:

Hi.
I'm a new customer.
Will [name of provider] be available at [time] on [date]?
Thank you!
[My name]
